01The psychiatry clinic day: before and after
Without one system, a psychiatry clinic typically keeps mental status examination on paper, appointments in a separate register, and billing apart from the patient record — so reception, the doctor and billing are each working from a different source for the same visit. On Yuktii, one visit record carries the reception queue, mental status examination and longitudinal notes, prescriptions and GST billing together, so nothing has to be re-entered or reconciled by hand between the front desk and the psychiatry doctor.
02Start with Patient Records
Most Psychiatry clinics begin with Patient Records and Mental Status Examination, then add Longitudinal Notes and Risk & Safety Notes once the team is comfortable. Appointments, prescriptions, GST billing and follow-up run on the same patient record throughout. For the full psychiatry examination, documentation and patient journey, see Yuktii for psychiatry.
03What the Psychiatry examination records
The structured mental status examination has 9 sections: Appearance & Behaviour, Speech, Mood & Affect, Thought, Perception, Cognition, Insight & Judgment, Risk Assessment and Rating Scales. Findings are picked from lists or typed in, and are saved on the visit record. Some sections record specific fields, for example mood & affect (mood (subjective) and affect (objective)), thought (form / stream, content and possession) and cognition (orientation, attention & concentration and memory).
04What is ready when you enable Psychiatry
When a clinic enables psychiatry in Yuktii, the app creates a structured mental status examination module, 100 quick-pick presets for complaints, examination findings and diagnoses, 8 prescription templates and 22 medicines. All of it is editable, so you can rename, remove or add to it to match how your clinic works. The presets speed up documentation; the clinician decides what is recorded and prescribed.
05Examples from the Psychiatry presets
Complaints you can pick from include Low mood, Anxiety, Excessive worry, Sleep disturbance, Insomnia and Irritability. Examination findings include Low mood noted, Anxious affect, Flat affect, Poor eye contact and Psychomotor retardation. Diagnosis labels include Major depressive disorder, Generalized anxiety disorder, Panic disorder, Insomnia and Adjustment disorder. These are starting labels to speed up note-taking, not clinical advice.
06Psychiatry prescription templates
The starter templates are Generalized Anxiety, Major Depressive Disorder, Panic Disorder, Insomnia, Bipolar Affective Disorder — Maintenance, Schizophrenia / Psychosis, Obsessive-Compulsive Disorder and Alcohol Withdrawal. Each is a pre-filled list the doctor edits before issuing, and the finished prescription can be downloaded as a PDF or sent on WhatsApp.
07Try it on real Psychiatry patients
The first 20 patients are free with the full Psychiatry workflow and no credit card, so you can run a genuine week of clinic before deciding. Solo then starts from ₹799/month and Pro from ₹1,499/month, depending on your specialty.
08Record the mental status the same way every visit
The mental status examination is structured, so a patient seen in March and in June is described in the same terms. Mood and affect, thought (form or stream, content, possession), cognition (orientation, attention and concentration, memory) and insight and judgment are picked from graded options instead of free text, and appearance, behaviour, speech and perception have their own sections.
09Risk assessment as its own section
Suicide risk, harm to others, self-neglect and absconding or wandering are each recorded on a graded scale (none, low, moderate, high, or none to severe for self-neglect) in a dedicated Risk Assessment section, so it is not buried in a paragraph of notes. The clinician makes the assessment; the section records it on the visit.
10Rating scale scores next to the visit
A Rating Scales section has fields for PHQ-9, GAD-7, YMRS and MMSE or MoCA. The clinician enters the score, so a change from one visit to the next can be read on the record.
11Medication changes across months
Eight starter prescription templates cover generalised anxiety, major depressive disorder, panic disorder, insomnia, bipolar affective disorder maintenance, schizophrenia or psychosis, obsessive-compulsive disorder and alcohol withdrawal. They are editable starting points the psychiatrist adjusts before issuing, and each issued prescription stays on the patient timeline.
12Follow-up for a long treatment
Psychiatric care runs over months, so follow-up matters. Recall lists show who is due, and reminders can be sent on WhatsApp; the wording of any message is set by the clinic, and access to records is limited by role.
