The case sheet brings together the patient's details (name, age and sex, blood group, phone, occupation, who referred them, address), the doctor, the treatment, the chief complaint, the history of the present illness, the examination findings and the diagnosis. Nothing has to be copied from one paper sheet to the next.
Allergies and medical conditions on the record appear as alerts at the top of the case sheet, along with pregnancy status and an ASA class when it is recorded, so a high-risk patient is flagged before the procedure starts.
The FDI tooth chart is stored on the patient's record, and changes are kept as a history, so the current state and the earlier state of each tooth can be compared instead of redrawing the diagram at every visit. The chart works for permanent and primary teeth.
The dental examination is structured into extraoral findings, intraoral soft tissue, salivary glands, and occlusion and other findings, with lists to choose from, so findings are recorded in consistent terms.
The case sheet PDF carries the treatment plan and procedures, prescriptions, lab work and the advice and outcome. Consent can be captured on screen for the treatment and stored with the record.
