Digital case sheet guide for dental clinics

A dental case sheet is the clinical record of one patient: who they are, what is wrong, what was found, what was done and what comes next. This guide lists what belongs on it, how a digital sheet differs from paper, and what to check before you switch.

The problem

Paper case sheets are slow and unsearchable

Charts get redrawn, old records go missing.

How Clinov solves it

Clinov carries the chart forward

An FDI chart stored on the patient record with its history, and case history shown at every visit.

What belongs on a case sheet

Identity and contact details, medical history and allergies, the chief complaint and its history, examination findings, the tooth chart, diagnosis, the treatment plan, what was done at each visit, prescriptions and advice, and consent. Anything a colleague would need to continue treatment without asking the patient again.

Flag medical risk at the top

Allergies, conditions such as diabetes or hypertension, and pregnancy affect what a dentist can do that day. A good case sheet shows them at the top, not on page two. Yuktii shows allergies, conditions and pregnancy status as alerts on the case sheet.

Chart it once, keep the history

On paper the tooth diagram is redrawn or overwritten at each visit, which erases the earlier state. In Yuktii the FDI chart stays on the patient's record and its changes are kept as a history, so you can see how a tooth changed between visits.

Record examination in the same terms every time

Free text varies between doctors and between visits. A structured examination (extraoral, intraoral soft tissue, salivary glands, occlusion) lets the findings be scanned and compared. Yuktii provides these sections with lists to choose from.

Make it fast enough to use

If completing the sheet takes longer than the treatment, it will be skipped. Presets for complaints, findings and diagnoses turn the note into a few taps; the dentist still decides what is recorded.

Before you switch from paper

Decide which fields you cannot do without, check that old records can come across (Yuktii's team imports records from an Excel or CSV file after review), and run a week of real patients side by side before dropping the paper sheet.

Common questions

What should a dental case sheet include?

Patient identity, medical history and allergies, chief complaint, examination, tooth chart, diagnosis, treatment plan, treatment done at each visit, prescriptions, advice and consent.

Are digital case sheets legally acceptable?

Records may be kept electronically. Retain them securely for the period your regulator requires and control access. Check the requirements with your dental council.

Does a digital tooth chart keep earlier versions?

In Yuktii, yes: changes to the chart are kept as a history.

Can I add consent?

Yes. Consent can be captured on screen and stored with the record.

Can I move my old paper or spreadsheet records?

Yuktii's team can import records from an Excel or CSV file after reviewing them, and an import can be rolled back if something is wrong.

Your data

Your data, your reports, your call

Everything you enter belongs to your clinic

Clinic and patient data is the clinic’s own. A clinic admin can download all of it whenever needed as one Excel workbook, with a sheet each for patients, appointments, treatments, prescriptions, billing, payments, inventory and staff.

Moving from another software?

Send us your existing records as an Excel or CSV file. Our team maps the columns, checks every row for problems and imports your patients, appointments, treatments, prescriptions, invoices and payments after review. An import can be rolled back if something is wrong. Talk to us about switching.

Reports you can download as PDF

Download the overview, today’s snapshot, revenue, expenses, patients, doctors, treatments, medicines, lab cases, stock and doctor commission as PDF reports. The analytics cover clinical, revenue, patient, doctor, inventory and operations views for any period you choose, with AI insights on top.
