General clinics: recording hypertension reviews and chronic follow-up

Hypertension is managed by repeat readings and repeat reviews, most of them in a general OPD. This guide covers a simple structure for recording BP and reviews, and how Yuktii supports it.

Buyer checklist

  • 01Record BP the same way at every visit
  • 02Open earlier visits to compare with today's reading
  • 03Book the next review before the patient leaves
  • 04Follow up patients who lapse

The problem

Hypertension often goes unrecognised, even within the same household

An analysis of NFHS-5 (2019–21) covering 636,699 households (BMJ Global Health, 2026) found that two or more members had hypertension in 14.9% of households, accounting for 49.8% of all hypertension cases, and that in 42.5% of those households none of the affected members was aware of it. A general clinic sees many of these patients, but a reading written on a slip cannot show that the same patient has been borderline for months.

How Clinov solves it

Readings, earlier visits and the next review on one record

Yuktii records vitals with every visit and keeps them on the patient record beside earlier visits, builds prescriptions from templates, and schedules follow-up and recall, so a review that is due shows up on a list.

1. Record BP the same way every time

Agree how readings are taken and recorded in your clinic, so numbers across visits and doctors are comparable. Vitals are recorded with each visit in Yuktii.

2. Compare with the last visit before the number

A single reading is a snapshot. Opening the earlier visits from the patient record shows whether a patient is stable, drifting or improving. Yuktii does not draw a BP graph.

3. Set the next review at the visit

Decide the interval by your own protocol and book it before the patient leaves, so the review exists on the diary.

4. Involve the household

The study above suggests hypertension clusters within families. Encouraging relatives to be checked, and recording family history, is a low-cost step within a consultation.

5. Work the overdue list

Each week, list the hypertension patients who missed their review and contact them. Recall rules and WhatsApp reminders in Yuktii make that list.

Common questions

Can Yuktii record BP over time?

Yes. Vitals are recorded at every visit and stay on the patient record. Yuktii shows each visit's values but does not draw a BP graph.

Does it alert on high readings?

Confirm any alerting you need in a free trial; this guide does not claim automatic clinical alerts.

Is there a free way to try this in a general practice?

Yes. The first 20 patients are free with the full platform and no credit card.

Your data

Your data, your reports, your call

Everything you enter belongs to your clinic

Clinic and patient data is the clinic’s own. A clinic admin can download all of it whenever needed as one Excel workbook, with a sheet each for patients, appointments, treatments, prescriptions, billing, payments, inventory and staff.

Moving from another software?

Send us your existing records as an Excel or CSV file. Our team maps the columns, checks every row for problems and imports your patients, appointments, treatments, prescriptions, invoices and payments after review. An import can be rolled back if something is wrong. Talk to us about switching.

Reports you can download as PDF

Download the overview, today’s snapshot, revenue, expenses, patients, doctors, treatments, medicines, lab cases, stock and doctor commission as PDF reports. The analytics cover clinical, revenue, patient, doctor, inventory and operations views for any period you choose, with AI insights on top.
